Incoming 7th Graders must have up-to-date Tdap immunizations. Central is offering a FREE Tdap clinic on Wednesday, July 27 from 9 a.m. to 3 p.m. Please return this authorization form to Central by Monday, July 18 if you would like to take advantage of that opportunity.
There is also an exemption form for those who object to vaccinations.
